 SPONSORSHIP AFFIDAVIT FROM INDIVIDUAL SPONSORING THE  CANDIDATE HOULD BE ON RS 10/- NON JUDICIAL STAMP PAPER 

1. I, No ________________ Rank _____________ Name _______________, Father/Brother of ________________(candidate’s name) do hereby solemnly affirm and declare as under that :-

(a) I am a permanent native of Village ______________Post Office ______________, Tehsil  _____________, Police Station ______________District _______________State _______________  PIN _____________.

(b) ______________ (Name of candidate), whose date of birth is ___________and joint photographs Affixed below is my real SON/BRATHER (As applicable).

(c) I am fully aware that, I can avail sponsorship facility under UHQ enrollment only for my One Real  SON/BRAOTHER  throughout my life.

(d) I have also not enrolled any of Son/Brother in the Army till date by using the benefits of my   Relationship Cert at ZROs/AROs/Regtl Centres.

(e) That now I have availed the sponsorship facility FIRST TIME under UHQ  enrolment for my real  SON/BROTHER (as applicable) named ________________for enrolment into the Army (Corps of  Signals) through UHQ recruitment during the recruitment rally ________(date) _____________(Regiment Center).

(f) I will not avail sponsorship facility under UHQ enrolment in future for my real  SON/BROTHER (As applicable)

2. I have personally checked all the relevant documents required for enrollment of my real SON/BROTHER into the Army as per the desired qualitative requirements and I certify that they are correct in all respects and nothing has been concealed. I understand that producing fake documents and giving false information at the time of enrollment will render termination of service of my SON/BROTHER despite any number of years service.

3. The above information is correct and the affidavit is being made for recruitment of my real SON/BROTHER in the Army. I f any contrary fact comes to light at any stage, I understand that I am liable for prosecution under the relevant law and my SON/BROTHER will be dismissed from service.
